Your First Visit

Patient Forms

Your first visit will begin with a warm smile and personal greeting! Our front desk and chiropractic assistants will greet you by first name unless otherwise requested. We have provided our new patient forms in the New Patient Center for your convenience.

This paperwork is our first insight to your overall health and spinal conditions and is vital to initiating your care. Please provide us with the best phone numbers and contact information so we may reach you to discuss your care. We ask that you plan to be in our office a total of two hours for your first in office visit.

If for some reason you will not be able to make your appointment, we request you notify us 12 hours in advance, so that another person in need of the doctor's time may be seen.


Consultation and Examination

The doctor will meet with you for a one-on-one consultation and examination of your spine and health status. We allot one hour for a face-to-face consultation and evaluation with Dr. Carmen Johanning or Dr. Cally Parks. The doctors schedules this time to focus entirely on you and your concerns. During this hour, she will take a thorough history and perform orthopedic, neurological, chiropractic and radiographic evaluations. The goal of this visit is to gather the needed information to determine the best care for your healthcare goals.


Radiographic Evaluation

Depending on your specific complaints and health history, we may evaluate your spine radiographically. X-rays help us develop the most effective treatment plan for you and allow us to see early changes in your spinal condition that have not yet created a painful condition. Often times we are able to locate the cause of your complaint AND identify spinal locations that have the potential to create more serious health concerns in the future. We will explain to you how the spine works and how the results of your examinations relate to your health. Most people are amazed once they see the condition of their spine on X-rays. Usually after the initial examination, they can even identify their misalignments and degeneration themselves.
On your initial visit, the doctors will briefly review their findings with you. After the doctors have had time to study and evaluate the information gathered on your first visit, they will schedule a more in depth explanation with you (Report of Findings).
